Plan members, providers and representatives join to form the MAC. Together, we come up with ways to improve:
Member medical care and our outreach plans
Member materials
Communication and care for members who speak a main language other than English
Phone services for members
Rides for members
It’s easy to join
We’re always looking for help finding better ways to do things. That’s why we try to make it as easy as possible to join the MAC.
Why join?
You can go to as many meetings as you’d like
You may be able to get rides to meetings
You may be able to get help with paying to travel to meetings, if needed

Any member of any age
Caregivers and local partners
How to join
You have a few options for joining the MAC.
By phone
Just contact us for more info. We’re here for you 24 hours a day, 7 days a week.
By mail
Just download the MAC form (PDF). Then, fill out the form and send it to the address on the form.
